PM Job Description:
Home Health Services of Virginia is seeking a reliable and compassionate LPN or RN to provide care for a patient in the home health setting. This is a part-time position with scheduled shifts on Monday through Friday.
Qualifications:
Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license from the Virginia State Board of Nursing. Experience working in a home health or home care setting preferred. Strong clinical, communication, and documentation skills .
Responsibilities:
Provide skilled patient care following the plan of care. Administer medications and treatments as ordered. Monitor and document patient condition accurately. Assist with activities of daily living as needed. Complete clinical notes in their entirety. Follow infection control and safety protocols in the home.
