Full Job Description Benefits:

Responsibilities:
Provide direct patient care, including administering medications, treatments, and procedures as ordered by physicians or advanced practice providers Monitor and assess patient conditions, documenting observations and reporting any changes to the healthcare team Collaborate with other healthcare professionals to develop and implement patient care plans Educate patients and their families on healthcare topics, including disease management, medication administration, and self-care techniques Maintain accurate and up-to-date medical records using electronic health record systems such as Epic Ensure compliance with infection control protocols and maintain a clean and safe patient environment
Requirements:
Valid LPN license in Virginia Three (3) years of experience working with adults who are diagnosed with a serious mental illness. Excellent communication skills to interact with patients, families, and healthcare team members Must be able to travel to clients' homes in the community as needed.
