Job Description

Southside Health Center provides medication-assisted treatment (including methadone and Suboxone) for patients who have opiate addictions. The LPN Dosing Nurse dispenses medication to our patients primarily in the outpatient methadone treatment program
- monitoring health status, documenting nursing notes, communicating with the Medical Director, and participating as part of the overall treatment team.

- 2303 Kalamazoo Ave As an integral member of the healthcare team, the LPN provides direct and indirect nursing care to patients as delegated by a Registered Nurse or provider.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Perform all delegated duties within the scope of education and validated competencies. Assist with patient flow and clinical support functions which may include scheduling appointments and managing external referrals. Take and record vital signs. Prepare for and assist providers with examinations and procedures, and carry out necessary clean-up. Obtain lab specimens, perform tests, and document results. Order and schedule lab tests. Administer medications per Agency guidelines. Complete clinical documentation per established Agency protocols. Participate in coordination of care which includes clinical data extraction and documentation at the direction of the RN or provider. Implement provider orders using established Agency protocols when applicable. Complete chart preparation for each patient including review of the population health management system, review and update incomplete orders and referrals, and gather other information that may be required such as Michigan Automated Prescription Services (MAPS). Record information in the electronic health record (EHR). Take phone calls, gather information, and provide information to appropriate staff for direction. Conduct follow-up calls in a timely manner, and provide education and instruction within scope of practice. Participate in Continuous Quality Improvement (CQI) activities as assigned. Understand and adhere to Agency compliance standards as they appear in the Corporate Compliance Policy, Code of Conduct, and Conflict of Interest Policy. Keep abreast of all pertinent federal, state and Agency regulations, laws, and policies as they presently exist and as they change or are modified. BLS/CPR training according to requirements of license and/or certification Understanding of the physical, emotional, and psychosocial needs of the patients Effective communication and interpersonal skills Ability to be self-motivated to work with and serve a diverse population Ability to work effectively and efficiently under tight deadlines and high volumes Working knowledge of computers and their applications Bilingual skills may be requiredCOMPETENCIES

Associates Degree or certification as an LPN. Licensed Practical Nurse in the State of Michigan and BLS/CPR certification required.EXPERIENCE
One year experience as an LPN with ambulatory care experience preferred, or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed above.PHYSICAL REQUIREMENTS
Possible exposure to patients/clients with illnesses and/or infections. Frequent telephone and computer use, requiring manual dexterity for writing and computer use. Sitting, standing or walking for extended periods of time. Specific vision abilities required by this job may include close vision, distance vision, and ability to adjust focus. May require bending, squatting, or kneeling and lifting up to 50 pounds. Use of Personal Protective Equipment (PPE) as required.CLIENT POPULATION SERVED
