36 Hours per Week Job Overview We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a 13-week Home Health assignment in the White River Junction, VT area. The ideal candidate will provide high-quality patient care in a home setting while collaborating with an interdisciplinary healthcare team to support patient wellness and independence. Responsibilities Provide skilled nursing care to patients in their homes under the supervision of an RN or physician Monitor patient conditions and report changes appropriately Administer medications and treatments as prescribed Educate patients and family members on care plans and disease management Maintain accurate and timely clinical documentation Assist with coordination of care and follow-up visits Ensure compliance with all agency policies and healthcare regulations Requirements Active Vermont LPN license in good standing 2 years' experience in home health setting. Home Health experience preferred Current BLS certification Valid driver's license and reliable transportation Strong communication and organizational skills
