Licensed Practical Nurse (LPN) responsible for providing patient care in a fast-paced outpatient setting, including rooming patients, triage, and clinical support under the direction of a provider. Ensures efficient patient flow while delivering high-quality, patient-centered care.
Responsibilities:
Room patients and maintain efficient patient flow for scheduled and walk-in visits Perform triage (in-person and phone) and prioritize patient needs per protocol Obtain and document vital signs, medical history, and clinical information in the EMR Communicate patient concerns and clinical findings to providers Assist providers with exams, procedures, and patient care Administer medications, immunizations, and treatments as ordered Process prescription refills and patient calls/messages Perform or assist with in-office testing and specimen collection Provide patient education and support for care plans and follow-up Maintain accurate, timely documentation and uphold confidentiality standards
Qualifications:
Active, unrestricted LPN license in Vermont Current CPR certification Strong clinical assessment and triage skills Excellent communication and interpersonal skills Ability to multitask and thrive in a fast-paced environment

University of Vermont Health•Porter Medical Center is located in the heart of beautiful Middlebury, Vermont. We are comprised of a 25-bed acute care facility, a 98-bed skilled nursing facility which includes a memory care unit, short-term rehabilitation unit and a long-term care unit, and a network of primary care and specialty medical practices throughout Addison County and the town of Brandon.
