Newport, VT — Skilled Nursing Facility (local candidates only) Responsibilities Administer prescribed medications and treatments following physician orders and facility policy. Monitor residents' health status, including vitals, pain, and mental status; report changes promptly. Provide direct patient care, including assistance with activities of daily living and mobility support. Document care provided in the electronic health record with accuracy and timeliness. Collaborate with RNs and the interdisciplinary team to implement and adjust care plans. Ensure safety and infection control practices; manage wound care and routine assessments as assigned.
