Job Description

Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality patient care across various settings, including long-term care facilities, hospitals, assisted living communities, and outpatient clinics. This role involves performing comprehensive patient assessments, administering medications and treatments, and supporting the overall health and well-being of diverse patient populations. The ideal candidate will possess a strong foundation in nursing practices, excellent communication skills, and a commitment to delivering respectful, patient-centered care. Duties Conduct thorough physical examinations and assess patient vital signs to inform clinical decisions. Administer medications, including injections and IV infusions, following established protocols and safety standards. Support complex medical procedures such as catheterization, wound suturing, and tube feeding. Assist with diagnostic evaluations, specimen collection, and processing to aid accurate diagnosis. Maintain detailed medical documentation in electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth. Provide specialized care for patients with chronic conditions such as Alzheimer's disease, dementia, or developmental disabilities. Manage acute pain episodes and participate in pain management strategies tailored to individual needs. Support discharge planning and coordinate with multidisciplinary teams for seamless patient transitions. Ensure compliance with HIPAA regulations to protect patient privacy and confidentiality. Collaborate with physicians, case managers, and other healthcare professionals to develop personalized care plans. Engage in ongoing education related to emergency medicine, critical care, geriatrics, or other specialized fields. Skills Proven experience working in diverse healthcare environments such as hospitals (including ICU and Level I Trauma Centers), nursing homes, hospice settings, or outpatient clinics. Proficiency with EMR/EHR systems including Epic, Cerner, eClinicalWorks, or Athenahealth. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and documentation standards. Skilled in performing venipuncture (phlebotomy), IV insertion, sterile techniques, and aseptic procedures. Experience managing complex patient cases involving infectious disease care or respiratory support like ventilator management. Ability to perform comprehensive patient assessments and physical examinations accurately. Familiarity with medication administration protocols including injections and IV therapy. Knowledge of infection control practices and safety procedures across various clinical settings. Excellent communication skills for effective patient education and team collaboration. Ability to work efficiently under pressure in fast-paced environments such as emergency rooms or trauma centers. Commitment to continuous learning in areas like geriatrics, pediatrics (including neonatal ventilator management), or behavioral health.
