Role Responsibilities:

Participate in developing and implementing patient care plans Provide skilled nursing care in the home setting Evaluate effectiveness of care and report changes in patient condition Accurately document medical history and clinical progress Provide emotional support and patient/family education Communicate clearly with patients, families, and care team members Perform additional duties as assigned

Qualifications:

Graduate of an approved school of nursing Current LPN license in the state of WA.
