Licensed Practical Nurse (LPN) - Home Health In this role, you will provide high-quality skilled nursing care to patients in their homes throughout the North King County area. Working under the physician's plan of treatment and in collaboration with patients and caregivers, you will support recovery, promote independence, and report patient concerns or changes in condition to the RN/Case Manager. Participate in developing and implementing patient care plans Provide skilled nursing care in the home setting Evaluate effectiveness of care and report changes in patient condition Accurately document medical history and clinical progress Provide emotional support and patient/family education Communicate clearly with patients, families, and care team members Perform additional duties as assignedQualifications:
Graduate of an approved school of nursing Current LPN license in the state of WA.
