Sea Mar Community Health Centers, a Federally Qualified Health Center (FQHC) founded in 1978, is a community-based organization committed to providing quality, comprehensive health, human, housing, educational and cultural services to diverse communities, specializing in service to Latinos in Washington State. ESSENTIAL DUTIES AND RESPONSIBILITIES
The LPN will: Coordinate closely with clients, providers, and interdisciplinary teams to deliver comprehensive care, including direct clinical services (e.g., medication administration, insulin management, wound care), and ensure timely implementation of all provider-ordered interventions. Conduct thorough assessments and maintain accurate clinical documentation, including monthly summary notes, evaluations, medication records, alert charting, and compliance with all documentation and quality control protocols. Develop, update, and implement individualized care/service plans in response to changes in condition, hospitalizations, or behavioral needs, including crisis interventions, medication refusals, and special precautions. Ensure compliance with clinical, safety, and regulatory requirements by adhering to infection control practices, emergency protocols, medication management standards, and reporting obligations (e.g., abuse, neglect, significant condition changes), while maintaining flexibility to meet staffing and operational needs. Education, Certificates, Licenses, Registrations, and Medical Screening The Crisis Center LPN must be licensed to practice as an LPN in the State of Washington, and has successfully completed the state mandated HIV education. LPN must obtain continuing education credits to meet licensure requirements. LPN must be licensed in the State of Washington to practice as a licensed practical nurse. Mental Health experience is preferred. This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment. Must complete a pre-hire and annual TB test screening. Annual employee health screening, tb test, annual influenza and Covid 19 vaccine required.
