Job Description
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) or RN(Registered Nurse) to join our team at Vibeyard park senior living Cottages Sequim. In this role, you will provide essential nursing care to our residents, ensuring their physical and emotional well being in a supportive environment.
Summary of Position and Essential Duties:
Responsible for planning, implementing and supervising resident services. Interviews, hires, evaluates and disciplines resident services staff. Establishes time schedules for resident services staff on specific shifts. Assigns and delegates responsibilities to resident services staff for the specific residents based on the qualifications of the staff and needs of the residents. Ensures each resident receives services prescribed or requested in accordance with federal and state regulations and facility policies and procedures. Assures prompt, appropriate action in cases of injury, illness, fire or other emergencies. In addition, serves as Marketing Ambassador in the ongoing effort to increase and/or maintain 100% census at all times.PERSONNEL
- Assist in the recruitment, interviewing and hiring of quality staff to meet staffing requirements while staying within budgetary constraints and without a need for temporary/agency help
- Counsel and discipline employees when required in a fair and consistent manner per policies
- Replace employees who call off for the next shift as directed
- Coordinate assignments with other support personnel. Provide a written assignment for each employee consistent with his/her experience and training which outlines residents to be cared for and other duties to be executed during the shift
- Supervise personnel and evaluate performance on an ongoing basis to ensure residents receive appropriate service
- Evaluate, discuss plan of action and follow through with specific problem on unit
- Assure documentation on resident records by unit personnel is complete and accurate
- Hold mini-conferences to keep personnel informed of new ends in resident service/care, policies and procedures
- Instruct personnel on use of equipment
RESIDENT SERVICE / CARE
- Complete service plan on new admission and update per federal requirements and state regulations and facility policies and procedures
- Participate in the development and implementation of resident service / care plans according to policy and procedure. Update as necessary. Engage resident and family participation in service plan planning by explaining approaches and involving them in teaching programs.
- Make resident rounds frequently to assure that each resident is given timely and appropriate service care including medication, treatments, diet as prescribed, nourishment, skin service/care to prevent pressure ulcers and is kept clean and comfortable. Observe resident symptoms, reactions to medications and/or treatments, changes in condition and unexpected needs.
- Notify administrator, physician and family and take appropriate action when significant changes and conditions occur. Write incident reports as indicated and follow up on incidents from other shifts as well as designated shift
- Prepare, administer and document medications and treatments as ordered by the physician in accordance with federal requirements and facility policies and procedures
- Order, receive medications from pharmacy. Contact physicians and record physicians verbal orders. Transcribe physician's orders with accuracy and assure follow up as indicated. Follow up stop order policy. Conduct monthly drug recap according to facility policy and procedure. County controlled drugs, dispose of discontinued medications according to state regulations.
- Monitor each resident's physical and mental functioning, psychological well being and nutritional status. To detect derivations from baseline and to gage progress. Obtain and document vital signs, weights, intake and output, diet supplements and replacements, etc. according to facility policy
- Ensure universal precautions are used properly throughout the facility. Maintain work area in clean, safe and sanitary condition at all times
- Ensure resident is treated with dignity and respect. Investigate complaints of residents and families and refer shift supervisor or take appropriate action
- Make necessary arrangements for consultants, laboratory and x-rays as necessary
FACILITY
- Maintain adequate supplies at resident services desk. Submit requests for supplies per facility system. Submit charge forms for all supplies used
- Assure resident services and equipment and work areas are clean, neat and orderly
- Complete forms and reports as requested by the Administrator
- Participate constructively in meetings and committees as requested by Administrator
- Attend in-service meetings
- Maintain good public relations with families, visitors, community members, residents and other facility personnel
- Respond quickly to all emergency situations.
