Job Overview Join our compassionate and dedicated team as a Full-Time Licensed Practical Nurse (LPN) in our skilled nursing facility! In this role, you will provide exceptional nursing care, support residents' health and well-being, and work closely with an interdisciplinary team to ensure the highest quality of care. Schedule:
Full-Time, Thursday through Sunday What You'll Be DoingResident Care & Well-Being Provide direct nursing care to residents in accordance with physician orders, facility policies, and state and federal regulations. Administer medications, treatments, and other nursing services as prescribed. Monitor residents' conditions and promptly report changes to the physician and supervising nurse. Assist with admissions, transfers, and discharges. Maintain accurate and timely documentation in resident medical records. Participate in developing and updating individualized care plans. Communicate effectively with residents, families, physicians, and other healthcare professionals. Staff & Team Collaboration Work closely with RNs, CNAs, therapists, and other team members to provide comprehensive resident care. Assist in supervising and directing nursing assistants during assigned shifts. Serve as a resource and support for nursing staff when needed. Promote a positive, respectful, and team-oriented work environment. Quality Assurance & Safety Follow infection control practices and safety procedures. Ensure compliance with all facility policies and regulatory requirements. Observe residents for signs of abuse, neglect, or changes in condition and report concerns appropriately. Participate in emergency preparedness activities and safety drills. Help maintain a clean, safe, and comfortable environment for residents and staff. Documentation & Compliance Complete all required charting and documentation accurately and timely. Maintain confidentiality of resident information. Ensure medication administration records and treatment records are completed correctly. Participate in surveys, audits, and quality improvement initiatives as needed. Professional Development Attend required in-service trainings and staff meetings. Maintain current licensure and certifications. Stay informed on best practices in long-term care nursing. Contribute to a culture of continuous learning and professional growth. Qualifications You'll NeedEducation & Experience Graduate of an accredited Practical Nursing program. Previous long-term care or skilled nursing experience preferred but not required. Certifications Current Licensed Practical Nurse (LPN) license in good standing. Current CPR certification. Experience with PCC preferred. Skills Strong communication and interpersonal skills. Excellent organizational and time-management abilities. Ability to work effectively as part of a healthcare team. Compassionate, resident-centered approach to care. Physical Requirements Ability to stand, walk, bend, lift, and move throughout the shift. Ability to lift and assist with moving residents as needed. Ability to respond quickly in emergency situations.
