The LPN works collaboratively with patients, families, physicians, therapists, and interdisciplinary team members to promote positive patient outcomes and exceptional care experiences. Essential Responsibilities Provide skilled nursing care in patients' homes according to physician orders and individualized care plans Perform patient assessments and report changes in condition to the supervising RN and physician as appropriate Administer medications and treatments safely and accurately Monitor and document patient progress, responses to treatment, and ongoing care needs Provide patient and family education regarding medications, disease processes, treatments, and self-care techniques Assist in coordinating services with physicians, therapists, caregivers, and other healthcare professionals Ensure accurate, timely, and compliant clinical documentation Participate in care conferences and interdisciplinary team meetings Follow all agency policies, infection prevention protocols, and regulatory requirements Support quality improvement initiatives and patient satisfaction efforts Qualifications Current Washington State Licensed Practical Nurse (LPN) license in good standing Current CPR/BLS certification Valid Washington State driver's license and reliable transportation Ability to travel throughout Spokane County Strong clinical assessment and communication skills Excellent organizational and time-management abilities Home health experience preferred but not required
