• Responsibilities:

• Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living (ADLs)
• Develop and implement individualized care plans, in collaboration with the interdisciplinary team
• Monitor and report patient progress, including identifying and addressing any concerns or changes in condition
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
• Maintain accurate and detailed records, including completion of patient charts and reports
• Participate in quality improvement initiatives and contribute to a culture of continuous learning and growth

• Requirements:

• Current LPN license in the state of Wisconsin
• Minimum of 1 year of experience in long-term care or post-acute setting
• Strong clinical skills and knowledge of nursing practices and standards
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
• Compassionate and empathetic approach to patient care
