Job Description

Greendale Park / Heritage Nursing & Rehab is seeking a caring and dependable Licensed Practical Nurse (LPN) to join our team! The LPN provides direct nursing care to residents under the supervision of an RN and assists with implementing individualized care plans. This role requires compassion, attention to detail, and a commitment to supporting residents' health and well-being. all shifts! full time or part time Responsibilities Provide safe and effective nursing care to residents in accordance with physicians' orders and care plans. Administer prescribed medications and treatments accurately and on schedule. Monitor residents' health, record vital signs, and report changes in condition to supervising RN or physician. Document all resident care, observations, interventions, and outcomes in medical records. Assist with admissions, transfers, and discharges of residents. Support and direct Certified Nursing Assistants (CNAs) in providing daily care and ensuring residents' comfort. Participate in resident care conferences and contribute to care planning. Maintain infection control standards, safety protocols, and compliance with state and federal regulations. Respond promptly and effectively to emergencies. Promote residents' rights, dignity, and independence. Qualifications Current and active Licensed Practical Nurse (LPN) license in WI Graduate of an accredited practical nursing program. Current CPR/BLS certification. Experience in long-term care or skilled nursing preferred, but not required. Strong communication, observation, and critical-thinking skills. Ability to work effectively as part of a healthcare team.
