Job Description

Licensed Practical Nuse, Home Health - Full Time Mercy Health Corporation $23.65 - $37.84 paid time off United States, Wisconsin, Janesville May 26, 2026 OVERVIEW The LPN performs nursing services that require the use of physical, social sciences, and nursing skills in the care of the sick, the conservation of health and the prevention of disease in a patient's home. Responsible for the care of the patients assigned as directed by the registered nurse. Other duties as assigned.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Performs skilled nursing care and collects patient data as ordered by the physician and directed by the registered nurse. Identifies and reports changes in condition to the registered nurse. Reinforces education provided by the RN and teaches basic principles of palliative and restorative care as well as chronic disease management Guides patients and families towards self-help in the recognition of physical, emotional, and environmental health problems. Teaches and validates patient and caregiver to ensure safe delivery of care. Maintains required records and documentation based on the care of patients and families. Documentation is submitted according to policy and ensures orders are kept current through communication with the registered nurse. Assists with implementing a plan for continuity of care that involves the patient, caregiver, other health care team members and community resources. Organizes, prioritizes and plans daily patient assignments effectively. Demonstrates proper use of PPE, hand washing, and bag technique to promote infection control and safety. Demonstrates understanding of safety in the community and work environment. Assumes accountability for assigned equipment (both medical and computer equipment). Participates in quality improvement activities. Attends appropriate continuing education in-services regularly and maintains professional competencies set by department.
EDUCATION AND EXPERIENCE
Graduate of an accredited
School of Practical Nursing OR Wisconsin:
Have completed the portion of the RN Program needed to obtain a certificate of completion in practical nursing.
Illinois:
Have completed the designated curriculum of a Ladder Option Approved Designated RN Program. A minimum one-year relevant experience preferred.
CERTIFICATION AND LICENSURE
Licensed as a Licensed Practical Nurse in state of practice Approved CPR or BLS certification required within 90 days of hire
ADDITIONAL REQUIREMENTS
Passing the Driver's License Check and/or Credit Check (for those positions requiring). Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check. Must be able to follow written/oral instructions.

Specific vision abilities required by this job include close vision, distance vision, color vision and ability to adjust focus.
