What You Will Do As a Licensed Practical Nurse (LPN), you will play a vital role in supporting the health, well-being, and quality of life of our residents by providing compassionate, person-centered care.
Responsibilities Include:
Provide direct resident care services while following individualized care plans. Administer medications in accordance with all standards of practice, state regulations, and community policies, including required documentation, reporting, and follow-up. Perform resident assessments as required by state regulations and community policies under the direction of the Director of Assisted Living. Observe residents for changes in condition and immediately report concerns to the supervisor and/or Director of Assisted Living. Document all pertinent observations, communications with residents' responsible parties, clinical procedures, and daily care activities by the end of each shift. Complete and maintain current individualized care plans for residents. Assist with other duties as assigned by leadership. Supervisory Responsibilities This position works under the supervision of the Director of Assisted Living and/or Director of Memory Care while exercising independent judgment and decision-making. The LPN may assist with the supervision, direction, and evaluation of other associates and may participate in performance reviews and disciplinary actions as needed.
Qualifications Required:
High School Diploma or GED. Current Licensed Practical Nurse (LPN) license in the State of Wisconsin. Current CPR certification. Minimum of one (1) year of related nursing experience. Strong communication, organizational, and resident care skills.
Physical Requirements:
Ability to sit, stand, walk, climb stairs, reach, bend, stoop, crouch, kneel, and perform repetitive movements throughout the workday. Ability to lift up to 25 pounds unassisted. Ability to assist with carrying, transferring, repositioning, pushing, and pulling residents using proper equipment and assistance as needed.
