  • 1650 weekly We are seeking a dedicated Licensed Practical Nurse (LPN) specializing in Long Term Care (LTC) to join our healthcare team.
In this vital role, you will provide comprehensive nursing care to residents in a long-term care setting, ensuring their safety, comfort, and well-being. The ideal candidate will possess a strong foundation in patient assessment, medication administration, and specialized care for individuals with chronic conditions, including dementia and Alzheimer's disease. Your expertise will support our mission to deliver compassionate, high-quality care tailored to the unique needs of each resident. Duties Perform thorough patient assessments, including vital signs, physical examinations, and physiological evaluations to inform individualized care plans. Administer medications and injections accurately, including IV infusions and other specialized treatments such as botulinum toxin therapy. Monitor residents for signs of acute pain, infection, or deterioration, and respond promptly with appropriate interventions. Manage complex care needs such as catheterization, tube feeding, ventilator support, and wound suturing while maintaining aseptic techniques. Document all patient interactions meticulously within Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations. Collaborate with interdisciplinary teams—including physicians, physical therapists, and case managers—to coordinate discharge planning and post-acute care. Assist with medical imaging procedures or diagnostic evaluations when necessary and support triage efforts for urgent resident needs. Provide specialized dementia and memory care for residents with Alzheimer's disease or other cognitive impairments. Support infection control protocols and participate in ongoing staff training related to healthcare standards such as NCQA or JCAHO. Engage in health coaching and education for residents and their families to promote optimal health outcomes. Experience Valid LPN license with current state licensure required; additional certification in Basic Life Support (BLS) is preferred. Prior experience working in long-term care facilities such as nursing homes or assisted living communities is highly desirable. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth. Experience managing complex cases involving geriatrics, dementia care, hospice services, or acute pain management. Knowledge of ICD coding (ICD-9/ICD-10), medication administration protocols, and medical documentation standards. Background in working with individuals with disabilities or developmental delays is advantageous. Exposure to hospital settings such as ICU or Level I Trauma Centers enhances clinical versatility. Familiarity with medical procedures like phlebotomy, venipuncture, sterile processing techniques, and specimen collection. Strong understanding of anatomy and physiology relevant to senior care populations.
