Job Description
We are seeking a part-time LPN to work every other weekend.
Position Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. This role is responsible for coordinating and overseeing resident health care services on a 24/7 basis in collaboration with the Director of Wellness. The LPN will provide clinical care, support resident service planning, supervise care staff, and ensure high-quality care for all residents.Key Responsibilities:
Resident Care & Services:
Provide direct nursing care, including medication administration, treatments, and health monitoring Conduct resident assessments, develop and update individualized service plans Communicate with physicians, families, and designated representatives regarding resident status and changes Respond to health emergencies and provide appropriate care within scope of practice Advocate for residents and ensure their physical, emotional, and social needs are met Maintain compliance with all healthcare regulations (HIPAA, OSHA, state regulations, etc.) Ensure high levels of resident satisfaction and promptly address concerns or complaintsCoordination & Oversight:
Supervise and support care staff to ensure quality service delivery Coordinate services with outside healthcare providers and ancillary services Assist with resident admissions, discharges, and transitions of care Ensure accurate documentation, recordkeeping, and physician order transcription Oversee medication ordering and treatment plansStaff Training & Leadership:
Train and mentor care staff in proper procedures and resident care Identify training needs and support ongoing staff development Promote a culture that supports dignity, independence, and quality of life for residentsAdministrative Duties:
Maintain accurate resident records and reports Ensure compliance with policies, procedures, and infection control standards Participate in on-call rotation and assist with scheduling needsQualifications:
Must be 18 years of age or older Active Licensed Practical Nurse (LPN) license in the state of employment Graduate of an accredited nursing program Minimum of one (1) year experience in home health, assisted living, or geriatric nursing preferred Strong communication and leadership skills Ability to work flexible hours, including weekends, holidays, and on-call shifts Ability to lift up to 50 lbs (30 lbs regularly) Current driver s license, reliable transportation, and auto insurance First Aid certification (or ability to obtain within 90 days)Additional Requirements:
