Job Description

Are you an LPN looking for your next role? We have the perfect job for you.
Position Purpose:
To provide direct patient care for patients of all ages requiring TBI rehabilitation and intensive medical management services under the direction of a Registered Nurse. To participate in the development of the interdisciplinary treatment plans or plans of care. To coordinate care for the patients served regarding presenting problems, and integrate areas in individual plans of care and update the plans as necessary under the direction of a Registered Nurse.
Key Responsibilities:
To provide leadership, direction, and supervision to CNAs.

To provide patient care routinely and gather data according to condition reports as indicated.

To provide for or assist with medication management and monitoring.

To gather data regarding nursing care and treatment progression using interventions and goals of the patients care plan .To assist with the nursing care of each patient under the direction of a Registered Nurse.

To assist in the intervention of crisis situations using approved Mandt techniques.

To actively participate in treatment team decisions, collecting data, verbalizing patient needs, assist in and document patient responses.

Adheres to safe work practices consistent with Lakeviews mission, Medial Staff Bylaws, Administrative, Clinical, and Human Resources policies and procedures.

To provide and document patient/family education.

To follow all state and federal regulations governing a hospital.
Qualifications:
1.
Skill, Knowledge and Abilities:
Working knowledge of patients care in both medical and rehabilitation patients. Ability to accurately administer medication and monitor side effects. Ability to delegate responsibility to other staff and monitor their performance. Skill in developing therapeutic relationships with patients and their families. Skill in preparing and maintaining appropriate medical record documentation.2.
Education:
Graduate of an accredited Licensed Practical Nurse school of nursing. CPR certified. ACLS Certified within 6 months of hire.3.
Licensure:
Current Wisconsin State License as a Licensed Practical Nurse.4.
Experience:
One year of experience with sub-acute or traumatic brain injury care for all age groups.
