Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across diverse settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will help ensure patients receive safe, effective, and personalized nursing services. Duties Administer medications and injections accurately following physician orders, including IV infusions and other specialized treatments. Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating physiological responses. Assist with medical procedures such as catheterization, suturing, tube feeding, and wound care using aseptic techniques. Maintain detailed medical documentation within Electronic Health Record (EHR) systems like Epic, Cerner, or Athenahealth to ensure compliance with HIPAA regulations. Support patient care activities in various settings including medical-surgical units, emergency rooms, ICU, PICU, and hospice care environments. Collaborate with interdisciplinary teams to develop discharge plans, coordinate case management efforts, and facilitate effective communication among healthcare providers. Provide education and health coaching to patients and their families on medication administration, disease management (including dementia and Alzheimer's care), and wellness strategies. Experience Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Demonstrated experience in diverse clinical settings such as hospital medicine, long-term care facilities, assisted living communities, or home & community care environments. Critical care experience in ICU or Level I Trauma Center preferred; experience with Level II trauma centers also valued. Proficiency with EMR/EHR systems such as Epic, Cerner, eClinicalWorks or Athenahealth for accurate medical documentation. Knowledge of
ICD-9/ICD-10
coding for diagnostic evaluation and billing purposes. Hands-on experience with IV insertion/insertion techniques including venipuncture and phlebotomy. Familiarity with infection control protocols, medication administration standards, and HIPAA compliance requirements. Experience working with individuals with disabilities or developmental challenges; background in geriatrics or memory care is highly desirable. Ability to perform physical examinations, vital signs assessment, specimen collection/processing, and basic life support procedures effectively.
