Job Description

LPN - Hospice Inpatient Unit / Home Hospice (Flexible Role) WV Caring is seeking compassionate and skilled Licensed Practical Nurses (LPNs) to join our growing team in the Elkins area. This unique position offers the opportunity to provide care in both our Hospice Inpatient Unit and our home hospice program, allowing nurses to practice at the top of their license while making a meaningful difference in the lives of patients and families. Position Summary The LPN will primarily serve as the third clinical team member in our Hospice Inpatient Unit during daytime hours. When inpatient census and patient needs permit, the LPN will support our home hospice program through patient visits, symptom monitoring, and coordination of care. This role is ideal for nurses who enjoy variety, teamwork, and building relationships with patients and families across settings. Schedule Primarily daytime hours Inpatient Unit coverage generally between 8:00 a.m. and 8:00 p.m. Home hospice visits typically between 8:00 a.m. and 4:30 p.m. Weekend rotation as assigned Full-time positions available Responsibilities Provide compassionate nursing care to hospice patients in the inpatient setting Monitor patient conditions and report changes to the RN and interdisciplinary team Administer medications and treatments as ordered Assist with symptom management and comfort-focused care Complete accurate and timely documentation Support patients and families through education and emotional support Perform home hospice visits when assigned Collaborate with physicians, nurse practitioners, social workers, chaplains, aides, and volunteers to ensure excellent patient care Qualifications Current West Virginia LPN license Valid driver's license and reliable transportation Strong assessment and communication skills Ability to work independently and as part of an interdisciplinary team Hospice, home health, long-term care, oncology, or palliative care experience preferred but not required Why WV Caring?
