Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. The ideal candidate will possess a strong foundation in patient care, with experience working with diverse populations, including seniors and individuals with developmental disabilities. As an LPN in this role, you will be responsible for delivering comprehensive nursing services, supporting patient well-being, and ensuring compliance with healthcare standards. Your expertise will contribute to creating a safe, respectful, and nurturing environment for residents requiring ongoing medical and personal care. Duties Administer medications and treatments accurately according to physician orders, including IV infusions and injections. Conduct thorough patient assessments, monitor vital signs, and document findings using electronic health record (EHR) systems such as Epic or Cerner. Assist with activities of daily living (ADLs), including mobility support, catheter care, tube feeding, and hygiene assistance. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to residents' needs. Manage acute pain episodes and participate in emergency response procedures, including airway management and ventilator support when necessary. Provide specialized care for residents with dementia or Alzheimer's disease, ensuring memory care protocols are followed. Maintain infection control standards through aseptic techniques and proper specimen collection/processing procedures. Educate residents and their families on health coaching topics such as medication management, nutrition, and discharge planning. Ensure compliance with HIPAA regulations by safeguarding patient confidentiality during all documentation and communication activities. Experience Valid licensure as a Licensed Practical Nurse (LPN) with current state certification required. Prior experience working in long-term care facilities, nursing homes, or assisted living communities preferred. Demonstrated proficiency in medical documentation using EMR/EHR systems like Epic or Athenahealth. Experience managing complex patient cases involving chronic conditions such as cardiovascular disease or infectious diseases. Exposure to various healthcare environments including hospitals (Level I Trauma Center or ICU), outpatient clinics, or post-acute care settings is highly desirable. Familiarity with
ICD-9/ICD-10
coding for accurate documentation of diagnoses and procedures. Knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation techniques essential for effective patient assessment. Skills in IV insertion, venipuncture, catheterization, and sterile processing are advantageous. Experience working with diverse populations including pediatric (neonatal ventilator), adult, elderly (geriatrics), or individuals with disabilities enhances your suitability for this role.
