Certification (massage therapists only) Professional license or certification (if required for your profession) Professional Liability Insurance Auto Insurance ID Card Auto Insurance Declarations Page W-9 Tax Form (completed during Stripe setup) Reliable transportation Professional equipment and supplies Ability to travel to client homes, hotels, vacation rentals, workplaces, and event locations How It Works Create your provider account through the i-thriv app or web portal by selecting "Become a Provider." Upload your documents, complete your onboarding quiz and orientation, connect your Google Calendar, set your availability, and finish your Stripe payout setup. Once your application has been reviewed and approved, you'll be ready to start receiving appointment requests. Simply accept or decline each booking based on your schedule and availability—you're always in control of when and where you work. As i-thriv continues to grow, so will your opportunities to receive more appointments and increase your earnings. Additional Requirement Before accepting your first appointment, approved providers must add i-thriv Wellness Anywhere LLC as an Additional Insured on their professional liability insurance policy.
