The Clinical Services Medical Assistant is responsible for assisting the care team in providing quality primary health care and ensuring timely response to patients and community partners under the direction of a licensed clinician. About CommuniCare+OLE Established in 2023, CommuniCare+OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

MINIMUM POSITION REQUIREMENTS
EDUCATION, EXPERIENCE, SKILLS/TRAINING Education:
High school diploma or general education degree (GED) required. Satisfactory completion of an accredited medical assistant program in
California Experience/Lived Experience:
CA Medical Assistant certification with a minimum of 2 years' experience required. Must certify and remain current in CPR. Experience working with patients and collaborating with multiple staff members to ensure optimal patient care. Experience in a community healthcare center preferred.
Special Skills/Training:
Strong interpersonal and customer service skills required. Computer skills; Microsoft Office Suite, strong data entry skills; electronic health record system preferred. Strong analytical and problem-solving skills and attention to detail required.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES 1.
Demonstrates exceptional customer service skills including greeting patients in a kind, compassionate and courteous manner; responds effectively to patient questions; manages multiple priorities and heavy patient workloads with patience and confidence. 2. Creates a patient centered environment that promotes our willingness to say yes to patients 3. Has a knowledge of insurance payors and state programs offered at CCOLE including but not limited to Partnership Health Plan, Medi-Cal, Medi-Care, Commercial Insurance, and Sliding Fee Scale 4. Strong communication skills and ability to effectively present and share facts and ideas both orally and in writing. Keeps BO supervisor, CSMA Lead, and any other appropriate department leads informed of pertinent matters. 5. Primary contact for answering provider backline calls and voicemails throughout the day and appropriately responding to community partners, including but not limited to hospitals, pharmacies, home health agencies, and specialty offices. 6. Acts as a community partner liaison facilitating the communication between external agencies and internal medical providers at CCOLE. Including but not limited to clarifying prescriptions with pharmacies and clarifying DI orders with imaging facilities 7. Serve as a back-up support for the medical assistant team, which includes all functions of a medical assistant, as directed by supervisor and medical assistant job description. 8. Point of contact to coordinate in person and phone triage with a licensed clinician (Registered Nurse, Pharmacist, Physician Assistant, Nurse Practitioner, Certified Nurse Midwife, or Physician) to ensure timely assessment of patient. 9. Under the direction of a licensed clinician, assist with ordering or recertifying durable medical equipment (DME) for patients. Will also review health records for completeness; searches for missing records; follows up with clinicians and other members of the care team to obtain missing documentation, information or requires signatures for DME processing. 10. Under the direction of a licensed clinician, facilitate the placement and/or clarification of external orders, including but not limited to, laboratory, imaging, and home health. 11. Documents all appropriate information in the electronic health record system ensuring to keep patient confidentiality. 12. Reviews and manages RN/Provider telephone/web encounters and documents, as appropriate and within MA scope of practice, to ensure timely communication with patients in collaboration with clinicians. Ensuring there are no duplicate telephone encounters or documents that may have been inappropriately routed to medical providers. 13. Facilitates the processing of prior authorizations for advanced diagnostic imaging orders 14. Assists local hospital staff in scheduling high priority discharge follow up appointments with primary care provider including initiating home health under the direction of a medical provider. Including obtaining records from community partners, including but not limited to laboratory, imaging, and hospitals, to facilitate timely, appropriate, and safe follow up for patients 15. Works closely with medical providers to provide high quality care to each patient by addressing their health needs and concerns 16. Primary contact for MyChart messages from patients by filtering to appropriate member of the care team or completing task that was requested within MA scope. 17. Closes the loop by scheduling appropriate follow up appointments with medical provider for patients who were triaged by FoneMed (outside nurse triage service) 18. Serves as first point of contact on the backline when receiving critical lab results from partnering laboratories, ensuring there is follow up from a medical provider. 19. Other duties as assigned
