Job Description
Come join our team! Renalus Center for Kidney Care is seeking a motivated and dependable CMA / Transitional Care Management Coordinator to join our dedicated team full-time. We are committed to providing the latest advancements in kidney care and take pride in offering exceptional service to our community. The Transitional Care Management (TCM) Coordinator is responsible for supporting safe and effective transitions of care for recently discharged patients while providing comprehensive clinical and administrative support to providers. This role focuses on timely patient outreach, care coordination, medication management, appointment scheduling, and communication management to improve patient outcomes, reduce readmissions, and ensure continuity of care. •CMA Certification required•The Coordinator works closely with physicians, clinical staff, and hospital partners to ensure patients receive appropriate follow-up care and meet Transitional Care Management requirements.
Key Requirements:
Monitor hospital dictation and discharge reports to identify eligible TCM patients. Contact patients within two (2) business days of hospital discharge. Conduct post-discharge assessments, including review of symptoms, medications, and barriers to care. Perform medication reconciliation and identify potential compliance issues. Document all patient interactions and complete the TCM Questionnaire in the EMR. Schedule post-discharge follow-up appointments within 7-14 business days. Obtain, organize, and scan hospital records into the EMR. Coordinate care with providers, hospitals, and support staff to promote successful transitions and reduce readmissions. Monitor and respond to emails, voicemails, and incoming faxes throughout the workday. Maintain accurate and timely documentation in the electronic medical record. Support providers with administrative and clinical tasks as needed.Education & Certification Requirements:
High school diploma or GED required. Medical Assistant diploma with active certification (this is mandatory and distinct from a diploma). Certification card must be valid and active.Preferred Skills:
Strong knowledge of EMR systems and documentation requirements. Excellent communication, organization, and time-management skills. Ability to manage multiple priorities in a fast-paced clinical environment. Commitment to patient-centered care and confidentiality standards.Additional Requirements:
