What You'll Do:

Serve as a key connection between patients and providers Monitor patient health status and follow up on provider-directed care plans Communicate with patients and families via phone, email, and in-person visits Help close gaps in care by coordinating services across various healthcare settings Support patients in managing long-term health conditions with empathy and precision

Preferred Qualifications:
