You are the liaison between the technology and our patients to support their engagement in our program and use of our vital devices to manage their chronic disease conditions. About the
Role:
As a medical assistant, you will service the patient, their provider, and the internal Olivera Health Group. Responsible use of their remote patient monitoring equipment and develop Patient-centered care plans. More specifically, you will be part of a new Olivera Health Group Team that will be accountable for managing our unique chronic disease population. You will work in a radically different model of healthcare. This role is in person, which means you will be based in the facility and service patients directly.
Medical assistant responsibilities:
Conduct vital taking with patients to review objective vital sign readings in our technology platform Escalate any findings out of parameters to the clinical facility staff. Conduct monthly facility reviews to ensure all facility-based eligibility patients are being serviced on our program. Expect collaboration, shared decision making, and partnership across clinical and non-clinical care team members, including in partnership with the patient's provider. Comfortable visiting patients at bedside to offer our services. Leverage strong time management skills to make impactful visits on patient care and daily balance of expected patient visits. Utilize our custom-built care facilitation platform, and the EMR to collect data, document member interactions in the field, organize information, track tasks, and communicate with your team, members, and community resources. Requirements for the
Role:
Certification from an accredited medical assistant program preferred Experience in patient based vital taking and readings.
