Job Description

Job description: Medical Assistant needed for a busy internal medicine practice. As a member of the physician office health care team, this professional assists with population health management through patient outreach and coordination of care, participates in the clinical visit to assist the provider, and provides self-management support to patients. Also assists with other office functions to support patient care and office operations.
Job Responsibilities:
Actively participates in population health outreach through identification of patients with gaps in care through use of analytic tools and patient registries. Follows all established protocols, policies, procedures and standardized work flows. Assists in managing data so that patients are effectively treated. Contacts patients and facilitates closing gaps in care. Ensures pre-visit planning is completed by performing chart and data reviews in advance of patient visit and preparing for patient arrival by identifying items that need addressed during visit. Demonstrates expanded rooming protocol by assisting the provider in addressing care gaps with the patient and appropriately activating standing orders. Performs registry and quality data review and conducts population outreach to close gaps in care. Assists in the identification of high risk patients and facilitates complex care management referrals. Works collaboratively with the care team to provide care coordination to rising risk patients. Works collaboratively with care team members to achieve population health management goals, to improve patient access to care and to reduce avoidable emergency department visits. Accurately performs clerical office functions or other duties as assigned or required.
REQUIRED
Knowledge, Skills, and Experience:
Qualified candidates will have at least two years of recent work experience, interest in population and data management, computer skills and can build spreadsheets. We are willing to train the right candidate with appropriate medical background. We are seeking an individual who can work both independently and collaboratively in a private practice setting. Knowledge in phlebotomy, EKGs, injections, triage and BLS preferred, but not required. Previous experience with EMR systems, AthenaOne is preferred, but not required.
