Yes Primary Purpose:
This position will work as a member of the primary care team to deliver comprehensive, person-centered services to the Portland community. Medical Assistants (MA) work to support providers and patients throughout the patient visit and are also integral members of the patient care team. MAs provide patient-centered care and work closely with providers and all clinical staff. MAs also work with front desk and administrative staff to ensure clinic flow is smooth and patient experience is positive. The Community Health Worker (CHW) will be responsible for helping patients and their families navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports an interdisciplinary team through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their family by providing social support and informal counseling, advocating for individuals and community health needs, and providing services as delegated by the interdisciplinary team. Section 2: Clinic Information The Center for Natural Medicine, Inc. is the first and one of the largest integrated healthcare facilities in the Northwest founded in 1991. The Center comprises a diverse team of healthcare practitioners including naturopathic physicians, chiropractors, acupuncturists, licensed massage therapists, and medical assistants working collaboratively to provide patients with well-researched and innovative treatments. Section 3: Description of Duties Medical Assisting 50% Manage patient flow, including but not limited to: Preparation for visits, including chart review for immunizations, screenings due, and data entry of pre-visit screening documents Preparing patients per standard workflow, including but not limited to: Collection, documentation, and reporting of patient's vital signs per clinic protocols. Completion of preventive screening questions Medication reconciliation with patients Administer injectable medications per provider order, with appropriate documentation according to clinic protocols. Perform venipuncture per provider order, with appropriate documentation according to clinic protocols. Perform CLIA waived tests per provider order, with appropriate documentation according to clinic protocols. Community Health Work 40% Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement. Providing ongoing follow-up, basic motivational interviewing and goal setting with patients/families. Follow-up with patients via phone calls and in office visits. Assist patients with completing applications and registration forms. Help patients set personal goals, and attend appointments. Suggest appropriate referrals to the interdisciplinary team. Help patients connect with transportation resources and give appointment reminders in special circumstances. Exhibit excellent working relations with patients, visitors and staff, effectively communicating Work cooperatively with other clinical personnel assigned to the same patient Be knowledgeable about community resources appropriate to needs of patients/families Be responsible for providing consistent communication to the interdisciplinary team Act as a patient advocate and liaison between the patient/family and community service agencies Meet documentation requirements as specified. Other 10% Exam room stocking and setup pre-visit and clean-up post-visit Maintain and clean supply rooms, exam rooms and MA workstations. Attend all quarterly all clinic meetings. Adhere to all state and federal privacy regulations, including HIPAA and to clinic policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information. Other tasks as assigned.
