CARE COORDINATOR / MEDICAL ASSISTANT
Assisted Living & Skilled Nursing Facility Settings | Theris, Inc.
OVERVIEW
Theris is expanding its clinical outreach team and looking for motivated, compassionate Care Coordinators and Medical Assistants to join us in bringing integrated behavioral health and specialty care directly to residents in assisted living communities and skilled nursing facilities. This is a field-based role where you will serve as the connective tissue between patients, providers, and facilities — making a tangible difference in the lives of some of the most underserved patient populations in your community. This is a paid position with a robust incentive structure. High performers have the opportunity to earn 1.5x to 2x their base compensation through our Attribution Bonus program — with no cap on earnings.
WHAT YOU WILL DO
Conduct patient intake and enrollment at assigned assisted living and skilled nursing facilities, including obtaining medical histories, documenting presenting symptoms, and coordinating with facility staff. Serve as the primary point of contact for residents being connected to behavioral health and specialty care services, including psychiatry, therapy, and dermatology. Coordinate scheduling, consent processes, and pre-visit preparation to ensure seamless provider visits at each facility. Maintain accurate, timely documentation in electronic health record (EHR) systems, including clinical notes, diagnosis codes (ICD), and care coordination activities. Communicate with providers, facility administrators, and payers to support authorization, eligibility verification, and continuity of care. Monitor patient engagement and follow up with residents and facility staff to support adherence to care plans and encourage ongoing participation in services. Assist with on-site logistics during provider visits, including room setup, patient escort, and post-visit documentation support. Identify new residents who may benefit from Theris services and facilitate the enrollment process in partnership with facility leadership.
COMPENSATION & INCENTIVE STRUCTURE
Base pay is competitive and commensurate with experience. In addition to your base, Theris offers a performance-based Attribution Bonus program tied to patient outcomes and continued engagement. Dedicated team members consistently earn 1.5x to 2x their base compensation through this program. Bonuses are paid on a rolling cycle and are designed to directly reward the quality and depth of your patient relationships — not just volume.
WHAT WE ARE LOOKING FOR
Prior experience in assisted living, skilled nursing, long-term care, or outpatient clinical settings strongly preferred. Familiarity with behavioral health populations, geriatric care, or care coordination is a significant advantage. Comfort working independently in a field-based environment with multiple facility locations. Strong interpersonal and communication skills — the ability to build trust with elderly residents, facility staff, and clinical providers is essential. Knowledge of medical terminology, basic clinical procedures, and EHR documentation. BLS/CPR certification preferred. Organized, reliable, and genuinely motivated by patient outcomes.

Pay:
$24.00 - $29.00 per hour Application Question(s): This is a health care position that will be paid hourly. However, there are incentive structures with commission in addition to the pay. Please explain if you have any prior history with sales or commission-based incentives
