Responsibilities:
Case Management and Care Coordination Answer intake calls for the case management line, gathering medical history anddocumenting triage findings. Coordinate patient care, including scheduling clinic appointments, obtaining medicalrecords, and faxing documents. Obtain and process release of information (ROI) requests for medical records. Clinical and Administrative Support Perform triage, obtain vital signs, and document medical histories. Maintain accurate and compliant documentation in electronic medical records. Educate patients about treatment plans, workplace safety, and medical standards. Medical Translation and Communication Translate medical records as needed. Provide Spanish language translation during patient interactions, includingtelemedicine appointments. Ensure clear, effective communication between patients, healthcare providers, and case managers.
Education and Experience Qualifications:
High School Diploma or equivalent (required). Completion of an accredited Medical Assistant program (preferred). Current Medical Assistant certification (preferred). Current Basic Life Support (BLS) certification (preferred). Spanish language proficiency (reading, writing, and speaking) highly preferred Experience in a clinical or case management setting (preferred). Experience with medical translation or interpretation (preferred).
