About Jade Internal Medicine Jade Internal Medicine is a private, membership-based direct primary care practice designed around a fundamentally different model of care—one that prioritizes time, continuity, and depth. We serve a small panel of highly engaged patients who value proactive, personalized medicine. This model allows for longer visits, direct physician access, and a level of follow-up and coordination that is not possible in traditional healthcare settings. This is a ground-floor opportunity to help build and operate a clinic defined by precision, responsiveness, and an exceptional patient experience. The Role This is not a traditional medical assistant role. As the Lead Care Coordinator, you will operate as the central coordinator of both the patient experience and clinical operations, working directly with Dr. Vu to ensure care is seamless, proactive, and highly personalized. You will function as a clinical operator + systems thinker, with ownership across patient communication, care coordination, and day-to-day clinic execution. Elevate the patient experience Protect physician time and energy Ensure operational excellence from day one Core Responsibilities 1. Patient Experience & Communication Serve as a primary point of contact for patients (text, phone) Ensure timely, thoughtful, and accurate communication Anticipate patient needs and follow up proactively Coordinate scheduling with a high level of flexibility and discretion Greet patients warmly, verify their information, and assist with patient intake processes to ensure a seamless check-in experience 2. Clinical Support Room patients and assist with visits Measure and record vital signs accurately while observing patient physiology and condition for any abnormalities Perform body composition analysis, EKGs, injections, point-of-care testing and other outpatient tests under supervision in compliance with safety protocols Assist with procedures (as applicable) Ensure exam rooms and clinical spaces are fully prepared and optimized by sterilizing equipment, arranging supplies, and ensuring a clean environment for patient visits 3. Care Coordination Track labs, imaging, referrals, and follow-ups Ensure no gaps in care or missed action items Coordinate with specialists, pharmacies, and external healthcare teams Support medication workflows, including prior authorizations when needed 4. Pre-Visit & Post-Visit Execution Prepare charts in advance (review history, flag key issues) Assist with documentation support and order tracking Ensure all follow-up items are completed and communicated 5. Operational Support Help build and refine clinic workflows Support medical administrative tasks such as managing patient flow Support onboarding of new patients Maintain organization of systems, supplies, and processes Identify inefficiencies and proactively suggest improvements Ideal Candidate 3+ years of experience as a Medical Assistant (primary care preferred) Strong clinical fundamentals and comfort with hands-on patient care Excellent patient service skills with a compassionate approach to outpatient care Clear, professional, and warm communication style Exceptional organizational skills and attention to detail High ownership mindset and accountability - sees what needs to be done and acts on it Comfortable with technology and modern communication tools Able to manage multiple priorities without losing precision Certification in Basic Life Support (BLS) or equivalent is required Mindset We're Looking For You anticipate before being asked You care about details others overlook You take pride in doing things exceptionally well You are calm, reliable, and solutions-oriented Compensation & Growth $28-38/hour, based on experience Performance-based bonuses Direct mentorship and exposure to a high-level care model Opportunity to grow into expanded operational leadership over time Embark on a fulfilling career where your skills make a real difference!
