Full-Time Job Overview:

Oasis Medical Group is seeking a compassionate, organized, and motivated Medical Assistant to join our Chronic Care Management (CCM) team. This role focuses on supporting patients living with chronic medical conditions through ongoing monthly telephone follow-up and care coordination. The Medical Assistant will conduct scheduled 20-25 minute phone calls with chronic care patients to help monitor their health, review medications, assist with care coordination, provide patient education, and ensure patients remain compliant with their treatment plans. Following completion of an initial 2-3 day in-office training period, the position will transition to a remote/work-from-home role.

About Our CCM Program:

Chronic Care Management (CCM) at Oasis Medical Group provides ongoing support for patients living with chronic medical conditions such as diabetes, hypertension, heart disease, COPD, and arthritis. Through regular follow-up, medication monitoring, care coordination, and direct access to the healthcare team, CCM helps patients stay healthier, prevent complications, reduce hospitalizations, and improve overall quality of life. Our CCM program ensures patients receive continuous, personalized care between office visits — helping them better manage their conditions, stay on track with treatment plans, and feel more supported in their healthcare journey.

Responsibilities:

Conduct monthly 20-25 minute CCM telephone calls with chronic care patients Build rapport with patients while providing compassionate and professional support Review medications and assess patient adherence to treatment plans Document all patient interactions accurately within the EMR system Identify patient concerns, symptoms, or barriers to care and escalate appropriately to providers Coordinate follow-up appointments, referrals, lab work, and preventive care services Educate patients regarding chronic disease management and healthy lifestyle habits Maintain HIPAA compliance and patient confidentiality at all times Communicate effectively with physicians, nurse practitioners, and clinical staff Meet daily productivity goals of approximately: 2 patients per hour 15 patients per day 5 days per week

Training:

Candidate will complete 2-3 days of in-office training at Oasis Medical Group Once training is completed successfully, candidate will transition to remote patient outreach and CCM management

Qualifications:

Medical Assistant certification preferred Prior experience in primary care, family medicine, internal medicine, or chronic care management preferred Excellent verbal communication and telephone skills Strong organizational and documentation abilities Comfortable using EMR systems (eClinicalWorks experience preferred) Ability to work independently in a remote environment Compassionate, patient-centered attitude with strong attention to detail
