Job Description

Overview The ACO Medical Assistant is a vital member of the ACO Department's multidisciplinary Care Management team, supporting the preventative care and chronic condition outreach programs for D-H attributed patients. Through patient engagement and EMR utilization, the ACO Medical Assistant acts as a liaison between the patient, care management team and payor to improve patient outcomes and our ability to meet value-based payment and performance activity targets. Part-Time, 20 hours/week. Monday
  • Friday 7am-11am.
You must have a current certification with the AAMA or registration with the AMT to be considered for this remote opportunity. Responsibilities Effectively partners with patients and care management team towards influencing shared goals. Facilitates patient outreach through phone call, patient portal messaging and/or mailings to schedule appointments, screenings and preventative tests. Effectively navigates and documents pertinent patient information within the Electronic Medical Record (EMR). Tracks and monitors patient outcomes related to preventative screening and chronic disease management, contributing valuable insights to interdisciplinary team discussions. Appropriately handles patient interactions, identifying actual and potential barriers to adhering to quality interventions involving nursing staff and/or department leadership as needed. Reviews gaps in care and disease registries to identify opportunities for outreach. Supplies authorized information to third parties upon request. Assists patients in completing forms associated with individualized care plans. Facilitates pre-visit planning, including patient reminders, instruction and preparation for specific procedures and/or warm handoffs as needed. Performs other duties as required or assigned. Qualifications Graduate of an accredited Medical Assistant Program required. Strong knowledge in Microsoft Office applications including Word, Excel, Outlook and PowerPoint required. Excellent interpersonal, customer service and communication skills, both written and verbal. Excellent time management and organizational skills. Ability to work independently as well as in a team environment.
Preferred Qualifications:
Previous experience in outpatient ambulatory care setting. Experience in supporting clinical quality programs e.g. HEDIS, Care Coordination or Chronic Disease Management. Ability to participate in role development and adapt to role changes as they evolve. Required Licensure/Certifications Certified Medical Assistant (CMA)
  • American Association of Medical Assistants OR Registered Medical Assistant (RMA)
  • American Medical Technologists OR Certified Clinical Medical Assistant (CCMA)
  • National Healthcareers Association plus DOL Registered Apprenticeships Certificate of Completion.
Current BLS certification required.
