Job Description

Street Health Outreach Medical Assistant Street Health Program At Father Joe s Villages, Street Health meets people where they are - in the places most systems don t reach. Our Street Health team provides outreach, engagement, and integrated healthcare support for individuals experiencing homelessness across San Diego County, helping people improve health outcomes and take meaningful steps toward stability and housing. As a Street Health Outreach Medical Assistant, you will play a unique hybrid role that combines, field-based outreach, care coordination support, and clinical medical assistant duties. You will meet patients in the community, build trust over time, and connect individuals to the healthcare system while supporting ongoing care in both field and clinic settings. What Success Looks Like Success in this role means building trusted relationships with individuals who are often disconnected from care and helping them stay engaged in services over time. You will work in dynamic environments - including streets, encampments, shelters, and clinics - supporting individuals with complex medical, behavioral health, and social needs. Your ability to combine clinical skills, outreach, and care coordination will directly improve patient engagement, continuity of care, and overall health outcomes. Maintain anassignedcaseload,including Enhanced Care Management (ECM) members, to support ongoing engagement, follow-up,and continuity of care Provide clinical supportincluding takingvital signs, documenting patient information, andassistinglicensed clinicians withpatientcare activities. Support care coordinationacross healthcare, housing,behavioral health,and social service systemsincollaborationwith interdisciplinary teams and community partners Completeaccurate,timelydocumentationfor outreach, clinical services, and ECM-relatedactivitiesin electronic health records Desirable Candidate Profile High School Diploma or GED required. Medical Assistant Certificaterequired. Minimum of one (1) year of experience in outreach, healthcare, case management, care coordination, or related direct service work withunderservedpopulations. Experience working with individuals experiencing homelessness, behavioral health conditions, substance use disorders, or complex health and social needs preferred. Experience providing ECM services preferred. Strong commitment to serving vulnerable populations with compassion, respect, and a belief in each person s capacity for stability and recovery. Basic computerproficiencyand ability to complete documentation in electronic systems. Valid California driver s license. Key Competencies Knowledge ofmedical assistant practices and clinical support procedures. Understanding of trauma-informed care, harm reduction,ECM,and whole-person care models Strong documentationskills and comfort using electronic health records Ability to manage an assignedcaseload across field and clinic settings Strong interpersonalskillswith the ability to build trust andengagediverse populations Ability to work independentlyin the fieldand collaborativelywithin multidisciplinary teams Commitment to delivering services with compassion, respect, empathy, empowerment, and dignity Work Schedule This is a full-time position involving field-based outreach and clinic-based work. Duties are performed across streets, encampments, shelters, and clinical environments. Schedules are typically Monday through Friday; hours may vary based on outreach schedules, patient needs, and program priorities. Compensation & Benefits The anticipated starting pay range for this role is $24.00 to $27.61 per hour. Final placement and starting rate will be determined based on the candidate's experience, qualifications, certification status, and internal equity considerations.
