In this role, you'll play a key part in improving clinical outcomes, supporting care coordination, and enhancing the experience for both members and their caregivers. This position is ideal for a compassionate MA who is passionate about improving the health and well-being of older adults through personalized, holistic support. Requirements Review Community Health Worker (CHW) appointment documentation for accuracy and completeness, escalating issues to the nurse case manager as needed. Support care coordination activities, including scheduling follow-up appointments and communicating with members or their caregivers. Mail and/or email patient care plans and educational materials as directed by case managers. Accurately update and maintain patient records within the electronic health record (EHR) system. Protect patient confidentiality and ensure compliance with all HIPAA and organizational policies. Experience, Skills, & Personality Certified or Registered Medical Assistant (CMA/RMA) required. 2 years of experience in a medical office, telehealth, or care coordination setting. Experience using EHR systems (eClinicalWorks preferred). Strong organizational skills and high attention to detail. Excellent written and verbal communication abilities. Self-motivated and able to work independently in a fast-paced, remote environment. Experience working with older adult populations preferred. Naturally empathetic, with strong active listening and problem-solving skills.
