- Combest Wellness Center Position Description Employees in this classification provide patient care management to an assigned clinic in the areas of clerical and clinical duties.

Optional Attachments Professional/Personal Reference, Professional License/Certification, Other Documents Supporting Qualifications Travel Required None Pay Grade Maximum 16.50 Major/Essential Functions Provides Care Management Services, refers patients and their families to resources that facilitate continuity of care, health promotion, and health maintenance and restoration. Reports data, identifies and reports alterations in patient responses to assist in the identification of problems and formulation of goals/ outcomes and patient-centered plans of care in collaboration with patients, their families, and the interdisciplinary health care team. Prepares and operates necessary equipment and instruments for examination, treatments, and procedures. Disinfects medical equipment and prepares instruments for sterilization. Assists with or performs exams and procedures and documents intervention in EMR (Electronic Medical Records). Takes and records accurate vital signs and measurements. Assists in determining the physical and mental health status, needs, and preferences of culturally, ethnically, and socially diverse patients and their families. Communicates patient data using EMR to support decision-making to improve patient care. Assists in collecting information regarding patients' physical and mental health status, and preferences of culturally, ethnically, and socially diverse patients and their families. Provides medical assistant support to OB/Gyn physician as delegated by the physician. Assists RN with ensuring all equipment used for OB/Gyn care is functioning and operational. Assists RN with monitoring all instruments and procedure trays to ensure they are appropriately sterilized and available for use. Grant Funded? Required Qualifications Education:
A minimum of a High School diploma or equivalent.License/Certification:
Graduation from an approved certified program for assistants in the area of required specialty. Certification by an approved accredited certifying agency in the specialty that particular work will be performed. Does this position work in a research laboratory?
