Job Description

COMMUNITY HEATH

ABOUT THE ROLE
The Community Health Worker (CHW) supports providers and the Care Management Team through an integrated approach to care management and community outreach. The CHW will be responsible for helping Care Managers support their patients and their families to access community services. Their goal will be to promote, maintain, and improve the health of both patients and their families by working directly with the Care Management Team.
FUNCTIONS OF THE POSITION
Works closely with providers and Care Managers to help ensure that patients have coordinated care. Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement, while advocating for appropriate services. Assist patients with completing applications and registration forms, finding transportation resources, and reminding patients of appointments. Facilitates healthcare and social service system navigation. Outreaches to patients to remind them of required testing, visits, etc. based on designated protocols.
SKILLS REQUIRED FOR SUCCESS
Medical Assistant/CCMA. Minimum three years' experience working in a healthcare role. Strong computer and organization skills. Ability to work independently, multitask, and respond to shifting priorities.
