We are seeking a compassionate and motivated Certified Medical Assistant Community Health Worker (CHW) to join our primary care team. The CHW works collaboratively and under the direction of the Care Coordinator to promote healthy lifestyle choices, provide patient education, and connect individuals to community resources. This role is key to supporting quality outcomes, smooth care transitions, and coordinated care across the health continuum. The ideal candidate is passionate about helping others make lasting health improvements, thrives on building community relationships, and is skilled at motivating and empowering patients to take an active role in managing their health.
Key Responsibilities:
Provide outreach and education to patients and community members managing chronic health conditions. Support the RN Care Coordinator in assessing participants' readiness for behavior change, identifying needs, and connecting them with appropriate community and health care resources. Deliver coaching and education that promote self-management and healthy lifestyle behaviors. Participate in health and wellness coaching sessions to help individuals achieve sustainable health goals. Monitor and document patient progress toward optimal wellness. Promote wellness and preventative care through education and engagement activities. Serve as a liaison between patients, providers, referral sources, and community partners to coordinate care and resolve continuity issues. Communicate effectively with health care team members to ensure efficient and coordinated care delivery. Assist in gathering medical records and relevant data from outside facilities. Conduct home visits to assess home safety and social determinants of health. Utilize electronic health record and registry data to identify care gaps and track patient progress.

Candidate:
WA State Certified Medical Assistant credential. Completion of Care Coordination, Health Assessments, Personal Safety and Professional Boundaries training within six months of hire. Valid driver's license, proof of vehicle insurance and reliable transportation for community outreach and home visits. Current CPR certification. Experience in community health, social services, patient education, or related field preferred. Strong interpersonal and communication skills with the ability to build rapport and trust with diverse populations. Knowledge of community resources and health promotion strategies.
