Summary of Responsibilities The Medical Coder is responsible for reviewing and validating OASIS assessments and clinical documentation to ensure accuracy, regulatory compliance, and reimbursement optimization within home health services. This role assigns appropriate ICD-10 codes, evaluates documentation to support physician-ordered care, and ensures compliance with Medicare, OASIS, and home health regulatory guidelines. The Medical Coder collaborates with clinicians, physicians, and interdisciplinary team members to clarify documentation, improve coding accuracy, support Plan of Care (POC) development, and stay current with regulatory updates. This detail-oriented position plays a critical role in maintaining accurate reimbursement, supporting quality measures and compliance standards, and promoting positive patient outcomes. Qualifications Minimum one (1) year of coding experience in home health or hospice, required. Working knowledge of CMS Conditions of Participation for Home Health Services, required. Strong experience with OASIS review, validation, and documentation requirements, including knowledge of current OASIS guidance and its impact on quality measures and reimbursement, preferred. Strong knowledge of ICD-10 coding, Medicare guidelines, home health regulations, and Plan of Care development, preferred. Proficiency with Homecare Homebase (HCHB) EMR, preferred. Strong attention to detail, organizational, analytical, and communication skills required.
