Job Description

Monadnock Family Services Description:
To provide direct psychiatric care to assigned consumers at various program sites. To include assessment/diagnosis for the purpose of determining appropriateness of medication treatment, ongoing medication evaluation/monitoring and participation in patient treatment planning as a member of the multidisciplinary clinical team.

Responsibilities:
Manages various teams and projects to ensure the highest level of standard of care adhering to medical best practices and informatics data. Ensures assigned consumers receive appropriate diagnostic assessment and treatment management in the content of a comprehensive treatment plan in collaboration with other team members. Provides continuity of care by collaborative involvement with other external care providers. Functions as an integral member of the multidisciplinary treatment team. Contributes to the overall Mission of MFS by participating in agency related functions. Provides direct psychiatric services consistent with MFS work standards. Completes administrative tasks to assure optimal availability and utilization of psychiatric time and services. Performs similar or related duties as assigned by Chief Medical Officer.
Qualifications:
NH Medical License or NH Nursing License and NH APRN License Current
DEA ABPN
