Care Manager - Emergency Department (ED)

Contract Position Overview "Navitas Healthcare, LLC" is seeking an experienced Care Manager - ED to support care coordination, utilization review, and discharge planning activities within a hospital emergency department setting. The ideal candidate will have strong clinical nursing experience and excellent communication and organizational skills. Key Responsibilities Coordinate patient care and discharge planning activities. Perform utilization review and care management functions. Collaborate with interdisciplinary healthcare teams to ensure appropriate patient care transitions. Maintain accurate documentation and compliance with hospital policies. Support patient flow and resource utilization within the Emergency Department. Utilize computer systems and electronic medical records effectively. Required Qualifications Minimum 5 years of clinical experience as a Registered Nurse (RN) .
Experience with:
Care Coordination Utilization Review Discharge Planning Computer literacy and proficiency with healthcare documentation systems. Associate Degree in Nursing (ASN/ADN) from an accredited nursing program. Active New York State Registered Nurse (RN) License . BLS Certification . PRI and Screen Certification (or ability to obtain within three months of employment). Primary Source Verification of RN License and Registration through NYSED. Preferred Qualifications Bachelor's or Master's Degree in Nursing. Leadership experience. Case Management Certification.
