Job Description

Job Summary:
The Primary Care Provider (PCP) will provide patient centered care primary care services utilizing the primary care team model of care to patients across the lifespan. The PCP will provide both technical and clinical advice to the PCP team in accordance with Best Demonstrated Practices, American Academy of Family Physicians Guidelines, and other applicable nationally recognized guidelines. The PCP will also collaborate with other Yellowhawk Departments to provide comprehensive care. The PCP will have some required administrative duties, which include supporting and being a part of leadership initiatives, attending meetings, and performing Quality Assurance and Improvement Projects. The PCP will develop and maintain relationships and partnerships that advance all of Yellowhawk's health initiatives. The PCP will report directly to the Chief Medical Officer. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.
Essential Job Functions:
1. Provide direction to assigned team (RN/MA's) in the treatment of care of patients. 2. Provides medical screening, evaluation, diagnosis, treatment and prevention services to all eligible patients in an outpatient clinic. 3. Documents all patient visits legibly and completely within two business days using a SOAP format. 4. Orders medically necessary diagnostic testing; analyzes and documents findings clearly within EHR. 5. Prescribes and/or administers medications; including controlled substances. 6. Orders referrals for outside specialized treatment when the patient's medical needs are beyond the limits of the medical facility and/or beyond the scope of the medical provider. Maintaining a positive collaborative relationship with outside medical practitioners and facilities. 7. Maintains records in compliance with professional, federal and other applicable guidelines; assures compliance with all accrediting agencies and with Yellowhawk policies and procedures. 8. Serves as a resource regarding medical issues to other Yellowhawk Departments. 9. Reviews all secondary referrals to ensure they are medically appropriate and within current Purchased-Referred Care priority guidelines. 10. Provides specialty services and clinics per program directives and needs. 11. Assists with the training and education of patients, Health Commission members, Yellowhawk employees and/or student interns. 12. Assists with departmental and organization accreditations; Patient-Centered Primary Care Home (PCPCH), Accreditation Association of Ambulatory Health Care Inc. (AAAHC). 13. Collaborates and develops relationships with internal departments. 14. Will report to the Chief Medical Officer or Medical Administrator any concerns regarding patient care. 15. Collaborates with the Chief Medical Officer and Medical Administrator on development and maintenance of standing orders, clinical policies, and quality assurance standards that are pertinent to the medical departments. 16. Participates in the development of departmental and Yellowhawk-wide annual work plans, progress reports, and monthly administrative reports as requested. 17. Maintains confidentiality in accordance with the Yellowhawk Tribal Health Center Policy Manual and the Health Insurance Portability and Accountability Act (HIPAA). 18. May be required to cover the duties of the Chief Medical Officer in the event of their absence. 19. Other related duties as assigned.
Minimum Qualifications:
1. Minimum Experience for
MD/DO:
Medical Degree (MD) or Doctor of Osteopathic Medicine (DO). Completion of an accredited Residency and board certified through the American Board of Family Medicine or American Osteopathic Board of Family Physicians. 2. Minimum Experience for
PA-C/FNP
Completion of an accredited program. Certified through their governing board and license in good standing with the State of Oregon. A minimum of three years in verifiable experience in Primary Care, Emergency Medicine, Urgent Care, Pediatrics or combination of experience that is equal to or greater than three years as a
PA-C/FNP
is preferred, however will consider highly qualified new graduates. 3. Current, active and unrestricted DEA registration. 4. Current, active, unrestricted state license as a Physician, PA, FNP; Oregon license preferred. Must obtain an Oregon license within 90 days of employment. 5. Electronic health record experience. 6. Represents Yellowhawk with a respected and viable presence in the community. 7. Demonstrated ability to communicate effectively, both in writing and orally, in any situation, with peers, staff, management, local, state and federal agencies and the general public, on various topics including, but not limited to policies, procedures and general operations. 8. Demonstrated skill in developing work processes, policies, and implementing quality improvement measures. 9. Effective ability to manage multiple tasks and projects in a fast-paced environment with multiple deadlines. 10. Ability to use tact, diplomacy, and solution-based thinking with all sensitive issues and special situations. 11. Demonstrated ability to research, analyze and synthesize documents and materials. 12. Demonstrated experience with Microsoft Office programs, EHRs, and/or other relevant software to meet the needs of the various programs. 13. Must possess and maintain BLS certification. 14. Must possess a valid driver's license and maintain insurance requirements to operate General Services Administration vehicles. 15. Must submit to and pass a reference and criminal background check and a pre-employment drug and alcohol screening.
Preferred Qualifications:
1. Experience and/or knowledge of history, culture and health needs of Native American communities. 2. Experience in a Tribal healthcare system. 3.
RPMS-EHR
experience.
