- Contract Company Overview Harmony Health Care Management is seeking a dedicated and experienced physician to fill the supervisory role of Hospice Medical Director .
In this role, you will provide comprehensive medical direction, oversee physician services, and ensure the highest standards of clinical excellence and regulatory compliance for our hospice agency.

Schedule:
24-hour routine availability for clinical oversight (shared/delegated as appropriate) Essential Duties & Responsibilities As the Medical Director, you will have overall responsibility for the medical component of the hospice program, serving as a medical resource and leading the interdisciplinary group.
Clinical Oversight & Patient Care Eligibility & Certification:
Review patients' medical eligibility for hospice services in accordance with established policies. Provide written certification of terminal illness for initial and subsequent benefit periods.
Face-to-Face Encounters:
Perform and attest to face-to-face encounters within thirty (30) days of the third and subsequent hospice benefit certification periods (or review findings reported by a Nurse Practitioner).
Plan of Care Management:
Establish, review, and update patient plans of care in conjunction with attending physicians and the interdisciplinary group at least every 15 days, or more frequently as needed.
Symptom & Medication Management:
Consult with attending physicians regarding pain and symptom management, and manage the oversight of patient medications and treatments.
Direct Care Availability:
Act as the primary physician for patients if their referring/attending physician is unavailable or wishes to relinquish care. Ensure competent physician services are routinely available 24/7 to meet general medical needs not met by attending physicians.
Clinical Documentation:
Document care provided in the patient's clinical record, providing clear evidence of the progression of the end-stage disease process.
PDF Supervisory & Quality Administration Regulatory Compliance:
Adhere strictly to the requirements, terms, and conditions of Medicare Conditions of Participation, accrediting bodies, and federal/state statutes.
PDF Policy & Protocol Development:
Establish, implement, and review policies, treatment protocols, and emergency procedures, including OSHA standards and the handling of hazardous medications. Quality Improvement (QAPI): Evaluate, monitor, and participate in Quality Assessment Performance Improvement (QAPI) teams and activities to identify medical education needs and assess cost/quality outcomes.

Interdisciplinary Leadership:
Participate in interdisciplinary group meetings, utilizing a team-oriented approach to resolve clinical, ethical, and interpersonal conflicts.
Community Liaison & Education Hospice Champion:
Represent and promote the hospice program to community physicians, physician groups, discharge planners, referral sources, health organizations, and potential donors.
Professional Liaison:
Act as a medical liaison with other physicians at Harmony Health Care Management and community colleagues, providing education on hospice and palliative care admission criteria.
Training & Education:
In cooperation with executive leadership, develop, review, and deliver training regarding the medical aspects of caring for terminally ill patients to physicians, staff, and volunteers.
Position Qualifications Education & Licensure:
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) licensed to practice in the state without restriction or history of disciplinary/corrective action.
Specialty:
Board Certified/Board Eligible in Internal Medicine or Family Medicine .
Registrations:
Must maintain an active, unrestricted controlled substances registration with state and federal authorities (DEA).
Experience:
Proven experience in hospice or palliative care, and/or specialized training in end-of-life care.
Medicare Status:
Must not be excluded from participating in the Medicare program.
Core Competencies:
Demonstrated knowledge in communications, resolving ethical concerns, and counseling patients and family/caregivers dealing with end-of-life issues.
PDF Continued Education:
Ongoing participation in medical education activities related to hospice and palliative care.
