Job Description

A growing, mission-driven healthcare organization in north-central Indiana is seeking a Primary Care Medical Director to lead and expand its outpatient primary care program. This is a unique opportunity to step into a true leadership role while remaining clinically active within an integrated care model. This organization has been serving its community for over a century and continues to expand its impact through a fully integrated model that combines primary care, behavioral health, and specialty services. With multiple locations and a large patient base, the system is well-positioned for continued growth and innovation. The Medical Director role offers a balanced structure of approximately 50% clinical care, 20% administrative oversight, and 30% program development . You will maintain your own patient panel while also helping shape the future of the primary care service line—overseeing workflows, mentoring advanced practice providers, and building out clinical operations. You will report directly to executive leadership and partner closely with behavioral health leadership, creating a highly collaborative environment focused on whole-person care. This is an ideal fit for a physician who wants to step into leadership, have a voice in program development, and make a meaningful impact in a community-focused setting. Position Details Flexible full-time schedule (40 hours/week); part-time options considered Opportunity for some remote administrative time Outpatient setting with integrated care model Leadership role with strong administrative support Qualifications Board Certified or Board Eligible Physician (Family Medicine or Internal Medicine) Prior leadership experience preferred (3-5 years ideal) Interest in community health / integrated care
