Opportunity:

Chrysalis Home Healthcare is looking for a physician partner who is passionate about improving patient outcomes, supporting safe transitions from hospital to home, and helping patients remain independent in their home environment.

Responsibilities include:

Provide physician leadership and clinical oversight Collaborate with our nursing and therapy teams Support quality initiatives and patient outcomes Provide guidance related to home health standards of care Participate in clinical discussions as needed

Ideal Candidate:

Texas licensed MD or DO Hospitalist, Internal Medicine, Family Practice, or Geriatric background preferred Understanding of medically complex patients and chronic disease management Interest in improving care transitions and reducing hospital readmissions
