We are seeking a highly qualified Medical Director (MD/DO) to lead clinical care across our Skilled Nursing Facility (SNF) partners in the Lufkin market. This role is ideal for a physician who wants more than just clinical work—you want leadership, ownership, and the opportunity to help build and scale a high-performing post-acute care model. This position offers strong earning potential, leadership growth, and the opportunity to serve as the primary physician leader within assigned facilities. This role may serve patients throughout Lufkin, Trinity and Livingston areas. Ideal candidates will reside nearby or be able to reliably commute to assigned facilities. This role includes:
Productivity-based earnings with strong upside potential Leadership opportunities tied to facility growth and performance Additional earning potential through APP oversight and facility expansion Long-term advancement opportunities within a rapidly growing organization Our model is designed for physicians who value autonomy, performance-based compensation, and leadership growth rather than a traditional capped salary structure. Compensation details will be discussed during the interview process based on experience, market, and scope of responsibility. Candidate QualificationsRequired MD or DO Board Certified or Board Eligible in Internal Medicine or Family Medicine Active, unrestricted TX medical license or ability to obtain licensure DEA license Ability to obtain facility privileges Strong clinical judgment and leadership capability Preferred 1-2+ years post-residency experience Post-acute care, SNF, hospitalist, or long-term care experience Experience leading APPs or interdisciplinary clinical teams Spanish fluency is a plus, but not required Candidate Responsibilities Deliver high-quality medical care to post-acute and long-term care patients Oversee multiple Skilled Nursing Facilities within the Reno market Supervise and collaborate with Nurse Practitioners and Physician Assistants Partner closely with Facility Administrators and Directors of Nursing Drive quality improvement and value-based care initiatives Lead documentation, compliance, and coding excellence Serve as the clinical face and trusted physician leader within assigned buildings Support census growth, facility relationships, and long-term strategic partnerships Improve patient outcomes while strengthening operational and clinical performance What We Are Looking ForCharismatic Leader You build trust quickly with patients, families, Administrators, DONs, and interdisciplinary teams.
