This position is supporting our home-based primary care program

It is located in an administrative office building. The Hartford HealthCare REACH program provides house call services by healthcare providers to patients 50 and older who are unable to leave their home without assistance or require a movement device (e.g. a wheelchair, walker crutches). Hartford HealthCare Medical Group's advanced practice providers (including physician assistants and nurse practitioners) will work closely with your primary care physician and other members of your care team to bring you high-quality healthcare in the comfort of your home. They can also become your primary care provider if you don't already have one. Job Description Serves as member of management team

• collaborates on organizational business issues, communicates appropriate messages, and supports and drives corporate initiative Oversees all activities of the assigned operational practice
• provides leadership and direction to direct reports and providers, and acts as positive role model Drives the development and execution of short
• and long-term business plans and corresponding budgets by collaborating with physician leadership and corporate administration in strategic planning and marketing for the practice Qualifications Bachelor's degree in business, healthcare management or a related field, or equivalent experience; advanced clinical or business credentials preferred Minimum 3-5 years medical practice management experience, preferably in a multi-site practice Epic Experience preferred.
