Job Description

We are looking for an experienced candidate with expertise in delivering quality and efficient customer service to patients and caregivers. The role entails overseeing a team of service coordinators through daily management, including training, coaching, problem-solving, and recognition, to ensure the team performs effectively and efficiently.
Primary Responsibilities:
Oversees the productivity and performance of the team responsible for coordinating patient services. Ensures ongoing assessments of departmental processes and procedures. You are also accountable for proposing strategies to enhance our operations, efficiency, and quality of service. Resolves issues with customer service, by identifying the root cause of the problem, proposing suitable resolutions, and swiftly implementing corrective action. Track and monitor admissions to hospitals and nursing facilities and maintain communication with patients regarding changes to their condition to ensure their overall needs are met. Compile and review documentation regarding patient, caregiver, caseworker, and/or the care team's communications, efforts to coordinate services, or complaints. Follows up with patients to guarantee their satisfaction with our services. Perform other related duties as assigned.
Qualifications:
Bachelor's degree required. Minimum of four years of experience in a healthcare supervisory position. Work experience in PAS/PHC Coordination preferred . Bilingual Apply today to join our team of dedicated patient care advocates!
