Job Description

JOB DESCRIPTION
Manage, maintain and organize the resident's medical record in compliance with federal, state, local requirements and facility policies and procedures. This position is a safety sensitive position as defined by applicable state law.
QUALIFICATIONS
: Must have experience in Medical Records or hold a License as a Licensed Practical Nurse or related field.
PHYSICAL REQUIREMENTS
Walking, reaching, bending, lifting, grasping, pushing and pulling, fine hand coordination, ability to understand and follow written and oral instructions and directions; ability to communicate with residents, families, outside agencies and consultants; and the ability to remain calm under stress.
RESPONSIBILITIES
Assuring resident safety. Must be on an on call rotation Maintain the medical record system in compliance with federal, state and local regulations. Maintain medical record information in a manner that prevents loss, damage or destruction. Assure that all medical record information is protected and kept confidential. Identify incomplete, late and/or inaccurate documentation and notify appropriate staff members. File medical records accurately. Develop system for prompt medical records retrieval upon request by authorized individuals and agencies. Notify Administrator of any medical records request immediately. o Process Medical Records Requests timely and accurately, with assistance of counsel. Answer correspondence regarding release of information from medical records according to facility policy. Prepare records in response to subpoenas or other legal process, with assistance of counsel. Maintain active Resident Medical Record including thinning the chart accordingly. Maintain and order necessary forms as pertaining to nursing documentation. Coordinate with physicians to ensure that medical records requiring their review and signature are available for their review and completed. Enter data into the computer timely, accurately and completely. Print forms and reports from the computer program as required. Take part in facility in-service programs as required. Travel to occasional meetings, conventions and seminars. These duties and responsibilities are not a comprehensive list, and additional tasks may be assigned from time to time. The scope of this position may change as necessitated by business demands.
